Welcome to Arcadia Health. To help your first visit go smoothly, please bring the following:

Photo ID and insurance card

List of current medications and dosages

Referral from your primary care physician (if applicable)

Prior cardiac test results or medical records (if available)

Completed new patient forms (available at our front desk or by request)

Insurance Accepted

We accept most major insurance plans including Aetna, Blue Cross Blue Shield, Cigna, UnitedHealthcare, Humana, Medicare, Medicaid, and TriCare. If you do not see your plan listed, please call our billing department at (901) 555-0180 to verify coverage.

Cardiac Emergency

If you are experiencing chest pain, severe shortness of breath, loss of consciousness, or symptoms of a heart attack or stroke, call 911 immediately. Do not drive yourself to the hospital. Our office is not equipped to handle acute cardiac emergencies — call 911 or go to the nearest emergency department.
